Multidisciplinary Diabetes Foot Clinics – for Patients with Active Foot Wounds/ Charcot Neuroarthropathy

• Patient will have an appointment with the Diabetes consultant and Diabetes Specialist Podiatrist to review the active foot wound

• A treatment plan will be discussed with the patient

• Patients will receive on going care which may be shared with the community podiatry team. either continue to be seen in the MDFT or referred to community Podiatry
