About the service
Practice teams play in important role in the diagnosis and management of patients with both pre-diabetes and diabetes. All diabetic patients should be offered diabetes annual review check.

B) Joint Diabetes specialist nurse / GP clinics
C) Link to general practice consultant visits

Carbohydrate Awareness Course
This popular course is run in the community and now within GP practices. This course is appropriate for patients who have received a diagnosis of Type 2 Diabetes over a year ago. These patients may have attended the DESMOND structured education programme in the past and looking to refresh their knowledge and awareness. The course focuses on teaching people how to manage their carbohydrate intake and lifestyle to improve their glycaemic control. The session is very practical and works with small groups to enable them to reduce their carbohydrate intake and weight as necessary.

Group Insulin Start Groups
These courses are run over 2 weeks by the DSN and Diabetes Dietitian and enable people to transfer onto insulin with support. The course aims to increase knowledge and practice with insulin treatment aiming to minimise weight gain and improve dietary control at this crucial time. In the second week the patient has support with their control and to gain experience to manage their own diabetes with insulin.
