About the service

We aim to deliver high quality specialist diabetes care in the community, the service focuses on patients with type 1 and Type 2 Diabetes in primary and community settings using the optimal use of resources and innovative models of care which aim to empower and promote patient self-management.

  • Diabetes education for our type 1 population [DAFNE] and Type 2 population [DESMOND] and carbohydrate awareness course

DESMOND

This is face to face education offered to all people within the first year of being diagnosed with Type 2 diabetes. It is provided at venues across South and West Herts only. It is provided as either a whole day session or 2 half days.

DESMOND for those unable to attend a face to face group session.

Patients who do not want to attend a face to face education session can choose from the following options:

  • Desmond booklet posted to them
  • MYDESMOND which is an online self directed course
  • If group education is not the best option then this can be provided on an individual basis, with use of the interpreter service if required

Carbohydrate awareness course

This popular course is run in the community. This course is appropriate for patients who have received a diagnosis of Type 2 Diabetes over a year ago. These patients may have attended the DESMOND structured education programme in the past and looking to refresh their knowledge and awareness. The course focuses on teaching people how to manage their carbohydrate intake and lifestyle to improve their glycaemic control. The session is very practical and works with small groups to enable them to reduce their carbohydrate intake and weight as necessary.
